City of Tulsa: Single-Family Residential Utilities CustomersAll single-family customers with regular, residential water service are automatically enrolled in the program. Vacant properties receiving no residential tap water and temporary accounts are not charged the $3.64 monthly fee. By participating, you and all permanent members of your household receive TotalCare benefits. This means you would not receive an EMSA ambulance bill if you or any member of your household needed emergency care. EMSA would collect any available insurance reimbursement, but would not bill you personally for any copayments or other costs related to emergency service that you'd normally have to pay. Please note: EMSA will send statements/explanation of benefit forms (showing no balance due from patient) after an ambulance transport. During the month of June, customers may opt out of paying the fee - thereby forfeiting benefits for the subsequent program year (July 1-June 30 of the next year). All members of your household would also lose benefits. If you opt out and then need emergency ambulance service, you may be billed directly for all or part of EMSA's billed charge ($1,100 plus $9 per mile). Customers who opt out also are personally responsible for paying full billed charges when insurance providers/Medicare deny EMSA claims. Per city ordinance, customers wishing to forfeit benefits for the coming program year may do so during the month of June by calling City of Tulsa Utilities Services at (918) 596-9511. June 30, 2008 is the opt-out deadline for the July 1, 2008-June 30, 2009 program year. An annual opt out is required.
